After you have submitted your order, you will need to complete the form below and submit it via email or to the address above, so that we can personalize the letter or certificate for each individual child (or adult). Some of the questions may not be relevant to each particular letter so feel free to complete the form to the best of your knowledge and we will take care of the rest by contacting you personally.

All information collected is kept strictly confidential and will be used only for the purpose of creating the letter or certificate.

Please be sure to complete a separate order form for each child you are ordering for. To place multiple orders, simply click "Continue Shopping" to return to the order form again.
